We know you do business with us because you trust us to stand behind our products and services. We appreciate your business, value you as a customer, and respect your right to privacy. We are committed to treat your personal information responsibly. The following privacy policy was created to show you we are working hard to protect your privacy.

HOW DO WE PROTECT YOUR INFORMATION?

We use those physical and technical safeguards available to us to protect your privacy. We restrict access to your information to only those who need it to perform their jobs for you. our business partners are also bound to use your information only for our purposes.

WHAT PERSONAL INFORMATION DO WE COLLECT ABOUT YOU?

We collect information we need to do our job from different sources.

•	We collect information you give us during conversations with our employees;
•	We collect information from the applications and forms you complete for us;
•	We collect information from your other transactions with us; and
•	Based on the product or service we provide, we may collect information from a consumer reporting agency, or medical provider.

WHAT DO WE DISCLOSE AND TO WHOM?

We do not sell any of your personal information to anyone. We may share this information with a business we use to provide you with product and services. We may disclose your information if we are required or permitted to by law. We may disclose your information if we required to for a legal or regulatory purpose. These include the following.

•	Information we receive from you on applications or forms we ask you to complete or we complete for you. This may include your name, address, Social Security number, drivers license number, and similar family member information. This may also include information about your personal property assets, and values.
•	This may include information from the insurance companies, brokers, and others we do business with. It may include your account balance, payment history, and claims history.
•	This may include information from a consumer-reporting agency or other reporting agencies. It may include your credit report, motor vehicle, and driving information on you or a family member.
•	We may disclose information to companies perform services for us under direct agreements. As permitted by law we may disclose some information to others with which we have agreements.

We do not sell your personal information. We do not share it except for the reasons described above. Therefore there is no need for an “op-out” process at The Sloan Agency.

We value you as a customer and want to be sure you are comfortable with how we use your personal information. Please call us if you have any questions.

We will provide a copy of any personal information we control and can easily retrieve. In order for us to do that we ask you to provide the following:

1.	All policy numbers for which you want access.
2.	Please provide a notarized request. This will insure the identity of the person making the request.
3.	We do not charge a fee for providing your information. However, we do reserve the right to charge a small fee in the future.
4.	You may request we correct information in your files. Please be aware we do not control information provide by third parties. Therefore you will have to contact if it is necessary to correct information they provide.
5.	Often we or a company we represent will request your consumer credit report. You may request the credit reporting agency’s name and address so you can ask the agency for a copy of your report.
